个人愚见，欢迎拍砖～同意LS，是95%的playground.因为前面已经说的很清楚了是调查playground，然后found后面接that引导宾语，95percent可以做代词主语，和one一样～

first of all, agree that D is correct.
2nd, I believe logically it should be: 95% of the playgrounds: 95% of the playgrounds in private child-care centers had unsafe conditions...
between playgrounds and child-care centers, i vote for the former because the survey was targeting at the former...

72 playgrounds的95%。 in private child-care centers 是72 playgrounds的定语。
