Q on the sending official record of your GMAT free of charge?

Could anyone who has taken GMAT tell me the format of the 5 schools that you can choose on the test date for sending test record free of charge? Shall I check for the codes of schools in Bulletin, and put them down on a sheet of paper for reference before test date? Or there are some choices of the states and schools listed in names from which you can choose? Which one? Anything except Confirmation No.and two forms of identification shall I take along with me?  Thank you very much![em27]
