感觉告诉我D 是对的,但是我无法rationalize

我也认为D是对的。B绝对支持；E大部支持。D中的迷惑点在于pharmacies ，他是指医院的药房，而不是指街边的。也就是说，病人从医院里那的药，并不是医生认为最有效的。

Hi,
B 在病人不能忍受医院通常使用的药时，许多医院就会使用Novex。
（这里的drug非Novex）
说明 For sinus pain 医院通常使用的药，很多病人不能忍受，可能是易起副作用之类原因（所以它们在街边买不到.），这时医院才会使用Novex。
在能被忍受的前提下，医院会首选其它药 the drug the hospitals usually use，这时 Novex不是the most effective painkiller for sinus Pain。
Novex的广泛运用，是因为它能被大多数人 tolerate，而非 most effective

juleyi 兄的分析确实好，但我认为你的分析也是基于很多你个人的假设，对于原文提供的信息我们无法推断。所以即使不是支持也只是个无关选项
