1。monitoring form 是什么东西？要在申请材料中附加的
2.examples of written work 是不是就是PS?还是别的什么？

1. I am not sure what is the monitoring form. But you can review the detailed content of the form.

2. means the writing sample which you wrote on a topic. For US univeristies, usually needs 15 pages at least.
