申请表上问每次工作的工资，不知是否必须照实际原封不动的填？

well, that's not a big deal if u count ur some other income such as housing from ur unit,ur stipend, ...into it. the most important thing is to make it sound reasonable.

When you apply, you needn't demonstrate your detailed salary.
