有的学校要求寄成绩单时，学校的登记员要填一张FORM，如果不填，只是有学校盖章的密封成绩单，有没有问题！！！

In my book, it is not a big deal. Last year, I also encountered such kind of problems. My schools didn't seal in the form. I only sent the transcripts and the form without seal.

那张form就不用填了，直接封寄成绩单就行了。
