Yes, it can include any scholarship /fellowship/assistanship.

you can first pay the deposit to save the seat and wait for greater news. as long as there are more schools admit you and you pay them deposit, you can definitely got more than one I-20.

no, just the school on-line application ask me to fill in my financial resources for I-20 consideration.

should not include fellowship at this stage as you have not gotten fellowship yet. If admitted and award fellowship, can include fellowship.
