please enclose this form with one official transcript /diploma( if your school policy allows)in the envelope provided. if no envelope is provided, please use your institution's standard business envelope and address to the applicant. please seal the envelope and sign across the envelope seal. return the sealed envelope to the applicant so itcan be include with his/her application. please do not send the transcript directly to the BS

2, what si the highest grade usually received?
3,what is the lowest grade usually received?
4,what is the average grade usually received?
