Instead of blaming an airline accident on pilot error, investigators should find out why the error was made by analyzing airplane design, airline management and pilot-training programs. For only then can changes be made to ensure that the same type of error does not recur and cause another accident.

问假设

(A) Pilot error is not a contributing factor in most airline accident. (为什么不对? 我觉得这个答案是weaken+not)

(D) Investigators of airline accidents should contribute to the prevention of future accidents.（怎么假设的呢？）

作者说investigator 不应该blame pilot,而应该积极地去find out the reason that caused the error to ensure that it doesnot recur. 如果没有investigator should contribute to prevention of the error的假设, 作者无法顺理成章地认为investigotor 应该去find out the reason instead of only blaming pilots. 如果A是答案的话,作者可能得出的结论是investigor 应该blame other factors 不一定要去find out.
