The school requests me to provide financial Statement together with my application.

Should I send the form with the Bank’s statement together with the application form or should I wait until receive the scholarship?

Will it affect my application for scholarship or assistantship if the amount of money is enough for the fees on Bank's statement?

you should submit this offical document with your apllication material to the school.

Doing this does not less your opportunity to obtain schoolarship from scholl.

One more question: Dose the amount of money on the statement has to cover all fees for two-year study? or just one year?

Except that the school(like CMU) especially stresses that you have to provide FS for two years, usually, you only need to prepare the funds for one year.
