我的理解，if only这里是副词，修饰because，意思还是“只要”。

这句的主干是Building large new hospitals in the bistate area would constitute

a wasteful use of resources, because the duplication of facilities should be

avoided。

if only起了个加强语气的作用。

只有duplication of facilities才能avoided

所以，只能在D和E中间看，D肯定不对，只有E，虽然看上去有点怪。
